Personal Information Notice

(Appendix to Covid-19 Harm Reduction Policy)

  1. A vaccination record and rapid test both contain personal information. Therefore, any collection, use, and disclosure must be in compliance with the Freedom of Information and Protection of Privacy Act (FOIP).
  2. The Division will not keep a copy of a vaccine record except to the extent necessary to confirm its validity and authenticity when required. Instead, the Division has contracted the services of ACESO Medical to collect all employee vaccine records.   
  3. ACESO Medical will provide the Division with regularly updated information regarding employee vaccination status.
  4. As per article 13 where a rapid test is required, the Division has contracted ACESO Medical to administer the required tests. Non-vaccinated, partially vaccinated or employees who have not disclosed their vaccination status will require, twice weekly, a rapid or PCR test through ACESO Medical. 
  5. In addition to the employee receiving their rapid and PCR test results, ACESO will simultaneously report the results directly to the Division.
  6. All information, provide by ACESO or the employee, will be housed in a secure location with access primarily limited to the Vaccination Coordinator/Human Resources. 
  7. If there is a breach of security affecting any employee’s personal information, including but not limited to vaccination status, the Division will immediately contact the affected employee. However, with the information being securely acquired and coded, the risk of improper use of personal information is low.
  8. If you need assistance in correcting or altering any information provided to ACESO Medical or the Division, including vaccination status, you are free to contact Human Resources.
  9. If you have any questions about the storage and safety of the information provided to the Division, you are free to contact Human Resources.
